You may scan & email forms, fax, mail, drop off in the drop off box at the Parish Office or drop off to the school office by Tuesday, August 21st, 2018. Your child will not be able to attend the first day of school on Wednesday, August 29th, 2018 if the school does not have these submitted forms.

Emergency Medical Form must be completed for each student.
Parent/Student Handbook Form must be signed by each student (all children in the same family can sign on the one form). You can read the Handbook here. By signing this form you accept this handbook and will follow the guidelines stated.
Students Acceptable Use & Electronic Device Policy Agreement Form must be signed by each student. This gives your student permission to use our computers and other electronic devices in the school building. You can read the entire Acceptable Use & Electronic Device Policy here.
Free & Reduced Lunch Form should be filled out by each family. Completing this application, whether you will qualify or not, will help us obtain federal funding for internet & technology, as well as, free breakfast and lunch services for the school. For students in grades 6th, 7th or 8th Grade who wish to bring a Kindle, Nook or Ipad to read on during Inside Reading Groups, you must have this Reading Contract signed.
