All the forms below must be returned by the first day of school. Please fill out the forms and email them to our school secretary.
General School Forms

- Preschool Enrollment Form
- Child Medication Form
- Health History Form
- Immunization Report (Only first page needed for PK, Can be separate form printed by your doctor’s office)
- Emergency Medical Authorization Form
- Child Medical Statement (Must be signed by Doctor)
- Pre-School Parent Communication Form
- Pre-School Child Release Form
- Photo Release Form
All students must also turn in a Birth Certificate & a Baptismal Certificate if your child has been baptized.
Kindergarten Forms

- Oral Assessment – signed by a Dentist
- Emergency Medical Form
- Parent-Student Contract
- Photo Release Form
- 2025-2026 Electronic Agreement
- Free & Reduced Lunch Form (We ask each family to complete this form even if you will not be ordering lunch. Every form we get helps our school get discounts on many products, especially Technology.)
